Referrals

If your child has been referred to a specialist by one of the providers here at PAL, please follow the following steps:


  1. Refer to your insurance book or call your insurance company to see if a referral is needed AND if any of the providers listed below accept your child's insurance plan.
  2. Call any of the providers listed below who are contracted with your insurance company and schedule an appointment.
  3. If a referral is required by your insurance company, please call our Patient Care Coordinator at 770-277-6725 ext 5 and leave the following information:
  4. Your child's name and date of birth
  5. Name of the provider with whom you made an appointment
  6. The date of the scheduled appointment 
  7. Our Patient Care Coordinator will complete the necessary paper/computer referral with your insurance company and send the appropriate medical information to the specialist with whom you are schedule.
  8. Please allow 48 hours for completion of referrals. Referrals can only be made for the same day appointments in an emergency.
